Use Cases

Two use cases were chosen as prototypes for the practical application of the TEAM-X data ecosystem. One is the use case “Medicine/Women’s Health”, in which women with breast cancer receive improved care across disciplines. The other is the use case “Digital Care Platform” for supply in inpatient and outpatient geriatric and palliative care.

TEAM-X is developing a communication tool that – taking into account the Gaia-X philosophy and the Digital Responsibility Goals – will accompany patients and physicians across disciplines at every step of breast cancer care. This goal will be achieved by further developing the GaiaONE solution to enable individualized accessibility of patient data on all end devices. This applies to all documents, from the doctor’s letter to findings and medical history forms. Tangential data such as quality of life questionnaires and vital signs are also included in this process.

The topic of breast cancer was selected as the “Medicine/Women’s Health” use case for TEAM-X. The goal of TEAM-X for breast cancer care is to accompany the care of women with breast cancer across disciplines in the use case “Medicine” with a communication tool for various steps of the breast cancer care process chain. The communication tool can be considered in terms of Gaia-X-compliant data collection and storage as well as data transfer to the treating physicians. Direct, low-threshold communication between patients and the medical team is thus made possible within TEAM-X. By using the TEAM-X end applications, it should be possible as an application-related goal to record documents from the clinic context by the treating physicians, but also documents or data on the state of health by the patients themselves, and to ensure the possibility of data exchange after release by the patients.

TEAM-X is developing a sensor system that collects patient data in the inpatient setting, currently primarily to prevent hazards and optimize nursing care. In the outpatient setting, a healthy client independently (personally or automatically) collects data about his health, activities, and preferences. In doing so, he or she has sovereignty over the further use of the data material within the TEAM-X data ecosystem.

A “digital platform for inpatient and outpatient care for the elderly” was selected as the second use case. The use case “Care” deals with outpatient and inpatient care for the elderly as well as outpatient and inpatient palliative care. The use case “Care” is defined here based on various use case stories, so that the technological and structural components can subsequently be aligned with the corresponding use case stories. As an example of such a use case story, the “Health Location Household” can be listed, in which family members or care providers enable the necessary outpatient nursing support. The goal of TEAM-X is to integrate new innovative digital technologies into the care processes through the data-related and care-related interfaces.
