The TEAM-X network pursues the following goals, for the implementation of which it develops strategies in close coordination with its eleven consortium partners and three cooperation partners.

TEAM-X has set itself the goal of strengthening the digital competence and innovative power of the health and care sector. Access to relevant health data enables a fact-based decision-making basis that empowers the players in the health and care sector to strengthen their digital competence and innovative power. However: Especially for small and medium-sized enterprises, it is a challenge to remain competitive and develop new business models in times of digital transformation. Not infrequently, there is a lack of financial means, time, and resources.

The trusted digital data ecosystem based on the Gaia-X infrastructure, which is being developed by TEAM-X, enables the creation of innovative data-driven business models, products, and services. These provide the basis for future-oriented cross-national healthcare.

Within TEAM-X, two use cases are initially being tested for this purpose:

 The use case “Women`s Health” is a communication tool that accompanies patients and physicians across disciplines at every step of breast cancer care, considering the Gaia-X philosophy and the Digital Responsibility Goals. To this end, the GaiaONE solution is being further developed to enable individualized access to patient data from all end devices. This includes all medically relevant documents. The goal of the “Women`s Health” use case is to establish a direct, low-threshold communication platform for patients and medical teams on which data from the clinic context can be exchanged. It also provides space for self-reporting of health-related data by patients.

In the use case “Digital Care Platform”, inpatient and outpatient care for the elderly is to be optimized by means of a digital patient image and the use of sensors. To this end, TEAM-X is developing a sensor system that collects patient data in the inpatient setting to prevent risks and improve nursing care. In the outpatient setting, a healthy client independently collects data about his or her health, activities, and preferences, with the client having the supremacy of forwarding the collected material within the TEAM-X data ecosystem. The technological and structural components of this digital care platform can be aligned with the appropriate use case stories. An example of this is the digital care platform – home health site. Through the resulting data-related and care interfaces, new innovative digital technologies can be integrated into the care processes.

“Your health, your data” with this guiding idea TEAM-X empowers citizens to handle their own health data in a sovereign, self-determined and barrier-free way. TEAM-X’s data ecosystem enables data sovereignty and ensures personal control. At the centre of TEAM-X’s activities is direct and easy access to one’s own health data in electronic form. This is what makes self-determined action and decision-making possible in the first place.

Transparency and control are basic prerequisites for data sovereignty in the collection, storage, use, and processing of one’s own data. Therefore, TEAM-X has set itself the goal of establishing a value framework that serves as an ideological superstructure of Gaia-X as a link to the global and European sustainability goals. For this purpose, the “Digital Responsibility Goals” developed by Identity Valley are established within TEAM-X and lived in the ecosystem. The Digital Responsibility Goals can be found at www.identityvalley.org.

The trusted digital data ecosystem to be developed by TEAM-X is intended to enable efficient exchange of health data not only for health care itself (primary use), but also for health research (secondary data use). The goal is to enable better use of health data in research, innovation, and health policy. To this end, TEAM-X establishes a sound legal framework for the use of health data for secondary data use.

TEAM-X thus promotes the provision of representative data for research and implements applications that – supported by AI – provide early detection of diseases in terms of personalized treatment and care. TEAM-X thus aims to strengthen Germany’s role as a leading science and business location.

For the use case “Women’s Health”, the topic “breast cancer” was chosen. TEAM-X is intended to achieve the goal of accompanying the care of women with breast cancer across disciplines with a communication tool through various steps of the process chain of breast cancer care. It can be made available – in terms of Gaia-X compliant data collection and storage as well as data sharing – to the treating physicians. For data collection, storage and release, data processing in terms of anonymization or pseudonymization must be ensured. Data sovereignty should always remain with the patient. Technically, this goal is achieved by TEAM-X specifically enhancing the GaiaONE solution to enable individualized accessibility. This is to be ensured by means of adaptive user interfaces for recording clinical documents – in pdf format as well as with photos of the patients. It will also be possible to record questionnaires on quality of life, side effects and vital signs. In the “Women’s Health” use case, GaiaONE will be used to store and synchronize all the patients’ end devices.

The “Care” use case deals with outpatient and inpatient care for the elderly as well as outpatient and inpatient palliative care. In the context of caring for clients in an inpatient setting, sensor systems (e.g., bedin, bedout, activity trackers) collect a variety of data whose current intended use is primarily to prevent hazards (e.g., falls). At the same time, data is generated that can be usefully exploited for good nursing care, but therefore comprehensively interferes with the privacy of the individual client. In the ambulatory care setting, a healthy client collects data about his or her health, activity, and preferences to make it available to the physician, the nursing service, or his or her support system as needed. This data is collected both manually and automatically on an individual basis. The client decides which data he or she wants to collect, which data he or she wants to release on a dataset-specific basis for which target group (doctor, counselor, relatives, therapist, research project) and for which occasion.

Der Use Case „Pflege“ beschäftigt sich mit ambulanter und stationärer Altenpflege sowie der ambulanten und stationären Palliativpflege. Im Rahmen der Versorgung von Klienten im stationären Kontext werden über Sensorsysteme (z. B. Bedin, Bedout, Aktivitätstracker) eine Vielzahl von Daten erhoben, deren derzeitiger Verwendungszweck primär in der Vermeidung von Gefahren (z. B. Sturz) liegt. Gleichzeitig werden Daten generiert, die für eine gute pflegerische Versorgung sinnvoll nutzbar sind, aber in der Konsequenz umfassend in die Privatsphäre des einzelnen Klienten eingreifen. Im Setting der ambulanten Pflege sammelt ein gesunder Klient Daten über seine Gesundheit, Aktivität und Vorlieben, um diese bei Bedarf dem Arzt, dem Pflegedienst oder seinem Hilfesystem zu Verfügung stellen zu können. Diese Daten werden sowohl manuell als auch automatisch individuell erfasst. Der Klient entscheidet, welche Daten er sammeln möchte, welche Daten er datensatzspezifisch für welche Zielgruppe (Arzt, Seelsorger, Angehörige, Therapeut, Forschungsvorhaben) und für welchen Anlass freigeben möchte.

Within the TEAM-X project, an incubator program is to be established as a platform for healthcare-related value-added service concepts with connectivity to the Gaia-X ecosystem. For this purpose, a two-phase incubation is to be carried out with a maximum of 20 selected digital value-added concepts from the healthcare sector. The goal of the incubator program is to develop viable concepts and minimum viable prototypes (MVPs) with connectivity potential to the Gaia-X ecosystem. The accompanied incubated teams thus complement the use cases within the framework of TEAM-X in a targeted manner and sustainably increase the relevance of TEAM-X to healthcare.

It can be seen that numerous business opportunities exist within the Gaia-X ecosystem. The development of new business models within the trusted digital data ecosystem of TEAM-X is therefore a goal. In addition to the technological issues, sustainable value creation networks are designed as part of the business model development.

Interlinking of the respective approaches (GaiaONE and GaiaCLOUD) and their further development into one product or one advanced smart service, respectively.
